How to Become an Empire Merchants Customer

We’re excited to start working with you! To open an Empire Merchants account you must complete the New Accounts Application below. If you would like this form faxed to you please call (718) 255-2641 and ask for the New Accounts department or email

Please note, the following information will be required of you when opening a new account:

A valid license issued from the New York State Liquor Authority.
A completed ST-120 form and a copy of the Certificate of Authority.
If you are an owner/officer of other establishments that hold New York State on-premise liquor licenses, please complete the Also Owns form.
In order for Empire Merchants to extend credit terms, a personal guarantee is required.

Upon receiving your New York State liquor license:

• Daily screening, which includes a temperature check and a health screening assessment is a requirement for all associates and visitors
• Social distancing must be maintained between individuals at all times
• Face masks or an acceptable face covering must be worn in common areas including lobbies and when traveling around the office.
Due to the circumstances related to COVID-19, you may experience longer than expected wait times when reaching out to our Customer Service and COD departments. Additionally, there may be a possible delay in your deliveries. We are working diligently to handle your orders and customer service needs as quickly as possible.
